All Sacrament preparation students must provide a copy of child/childrens BIRTH and BAPTISMAL certificate at the time of registration; Copies of your child's/children's birth and baptismal certificates can be summitted to: Church of the Holy Spirit, 1800 E. Libra Drive, Tempe, AZ, 85283, or brought to the Religious Education Office.
